Do I need a prescription from my doctor to be evaluated for a prosthesis or orthosis?
Yes, but if you do not have one, it's usually not an issue for us to obtain a phone order for an evaluation, as long as the physician is aware of the need for the device.
Does my insurance pay for prosthetic and/or orthotic services?
There are a lot of factors that determine if a device is covered by a specific policy. The short answer is most policies either partially or fully cover prosthetics and orthotics. You may need a referral and/or prior authorization. Contact your insurance company to fully maximize your benefits. Once we have seen you for an evaluation, we can help with this process.
What insurances do you accept?
We accept most Private, Medicare and Medicaid Insurers. Feel free to contact our office to determine if we accept your insurance, or complete our contact us form.
I recently had an amputation, how do I know when I’m ready for fitting?
In general, once your amputation site is completely healed, you are ready to begin the fitting process. It's oftentimes beneficial to come in before this to discuss the prosthetic rehabilitation process, so you will be prepared.
What do I need to bring to my first appointment?
Please arrive 10 minutes early for your first appointment to allow time to complete the paperwork. Remember to bring your most recent insurance card with you, as well as a prescription from your doctor (if you have one). If you do not have a prescription, we will need to obtain a phone order to conduct the evaluation.
